You will receive an email after registration has been submitted confirming your reservation into the camp.

Payments can be made the first day of camp by Visa, Cash , or Cheque payable to Olympic Hockey Academy Inc.

Parent Consent and Liability Waiver

The Participant and parents acknowledge and agree that the Olympic Hockey Academy or any of the principals, officers, employees, agents directors or instructors will NOT be held responsible for any accident damage, injury or loss, however caused, negligent or otherwise, at any time and expressly releases any and all the aforementioned parties from all claims arising from any accident, damage, injury or loss or as a consequence thereof.  Also, I hereby authorize the Olympic Hockey Academy staff to seek any emergency medical treatment it deems necessary for my son, daughter or legal dependant. I authorize the use of any photos taken at Olympic hockey program for promotional purposes.

 

 

Cancellation Policy

Full Refunds (less $50.00 administration fee) are available for cancellation made at least 30 days before the start date of any camp.  Refunds (50%) are available for any cancellation made 30 days or less before the start of any camp due to serious injury or illness (a Doctor’s letter is required)

No credits are issued for missed classes.

No Refunds will be issued for private / semi private lessons.

There is a $25.00 service charge for NSF cheques.
